Introduction

Healthcare in Africa is evolving. Infectious diseases, non-communicable diseases, pandemics, and emerging health threats have reshaped how care is delivered. One profession that sits at the center of this transformation is Medical Laboratory Science which is a part of the allied health sciences, alongside fields like physiotherapy, pharmacy, radiography, and nursing [1]. These are academic-based professions that require strong science backgrounds and structured training. 

 

Who is a Medical Laboratory Scientist?

A Medical Laboratory Scientist is a trained healthcare professional who performs diagnostic tests on body fluids and tissues. These include blood, urine, stool, sputum, cerebrospinal fluid, and other specimens [2]. Working to support the detection, diagnosis, monitoring, and treatment of diseases [1]. Medical laboratory scientists do not simply “run tests.” They analyze, interpret, validate, and ensure the accuracy of results before those results reach clinicians. The analytical work they perform depends on a solid understanding of medicine [2].

Medical Laboratory Scientists work across five major laboratory disciplines:

  1. Clinical chemistry
  2. Hematology and Immunology
  3. Transfusion science 
  4. Microbiology
  5. Histopathology 

In many African countries, the professional title “Medical Laboratory Scientist” is legally protected. This means individuals must complete accredited training, undergo internship, meet ethical standards, and be licensed before practicing. Regulatory councils oversee training standards and practice guidelines to ensure patient safety.

Importantly, Medical Laboratory Scientists are not subordinate to other professions. In several African countries, professional reforms and regulatory acts have strengthened their autonomy and recognition [1].

Simply put, if doctors are the detectives of medicine, Medical Laboratory Scientists provide the evidence.

 

What do Medical Laboratory Scientists Do?

Medical Laboratory Scientists are responsible for generating accurate and reliable test results that guide medical decisions. Their work includes:

4. Quality Control and Assurance

5. Public Health Surveillance

6. Technology and Innovation

7. Education and Guidance

How are Medical Laboratory Scientists Trained and Licensed in Africa?

Across many African nations, the most common entry pathway is a Bachelor of Medical Laboratory Science (BMLS), which typically lasts 3–5 years, depending on national university policies and whether internship or research thesis components are included [3]. The training integrates:

  1. Classroom-based foundational sciences
  2. Practical laboratory sessions
  3. Supervised clinical rotations in teaching hospitals or reference laboratories

Most African countries require a structured internship period after graduation. A 12-month internship is widely recommended as the minimum standard. Some other African countries operate longer supervised training models before full registration.

For mid-level cadres, several countries offer diploma or technician-level programs lasting between 6 and 24 months. For example:

  • Kenya offers Medical Laboratory Technician courses
  • Tanzania runs two-year Laboratory Technologist programs
  • Ghana, Nigeria, and Zambia have 12–27-month structured courses [3].

Regional harmonization efforts are ongoing. The World Health Organization Regional Office for Africa (WHO-AFRO) developed competency-based curricula to support standardization. The East African Community (EAC) also established common curricula and joint examinations to promote mobility between Kenya, Uganda, Tanzania, Rwanda, and Burundi. Also regional bodies such as the West African Postgraduate College of Medical Laboratory Science (WAPCMLS) support postgraduate specialization and collaboration across countries.

Salary levels for Medical Laboratory Scientists vary widely across Africa depending on:

For example, wage comparisons between Nigeria and the United States show major differences, which have contributed to professional emigration [1]. Similar patterns are seen across other African nations where professionals migrate to Europe, North America, and the Middle East. Brain drain is a serious issue. One study reported the loss of 906 medical laboratory scientists in a single year due to migration [1]. This pattern affects multiple African countries, not just Nigeria.

As healthcare systems strengthen and laboratory infrastructure expands across Africa, MLS professionals are finding opportunities in increasingly diverse settings.

Graduates can pursue careers across multiple sectors, including:

  1. Clinical and hospital laboratories, where they perform diagnostic testing across specialties such as hematology, microbiology, clinical chemistry, transfusion science, and histopathology
  2. Public health and reference laboratories, supporting national disease surveillance, outbreak investigation, and epidemiological research
  3. Research institutions and universities, contributing to laboratory-based research, curriculum development, and academic training [3]
  4. International organizations such as UNICEF, WHO, and CDC Africa, where laboratory expertise supports global health programs and emergency response
  5. Military and paramilitary health services, providing diagnostic support within national security health systems
  6. Industrial, pharmaceutical, and biotechnology sectors, where laboratory scientists contribute to quality control, product development, and regulatory compliance
  7. Private diagnostic entrepreneurship, through the establishment of licensed diagnostic centers, which expands healthcare access and creates employment [2]

Postgraduate specialization is also an expanding pathway. Regional bodies such as the West African Postgraduate College of Medical Laboratory Science (WAPCMLS) support advanced training and professional development across countries, enabling MLS professionals to deepen their expertise in specific disciplines.

As laboratory medicine becomes more automated and technology-driven, professionals who continuously update their skills in molecular biology, bioinformatics, quality management, and research methods will have broader and more competitive opportunities [1]. An indication that MLS is a dynamic profession with pathways spanning clinical practice, research, policy, academia, entrepreneurship, and international health.

 

Accreditation and Regulation

South Africa currently leads in accredited training programs, with 23 recognized programs [3].

Nigeria strengthened regulation through the Medical Laboratory Science Council of Nigeria Act, which repealed earlier decrees and established clearer licensing frameworks [1].

Professional councils in various countries are responsible for determining minimum qualification standards, conducting licensure examinations, maintaining professional registers, enforcing codes of conduct and monitoring continuing professional development.

However, weak regulatory frameworks in some countries have led to variation in training duration, certification standards, and scope of practice [3]. The lack of strong professional bodies in certain regions has allowed questionable private institutions to operate without consistent quality oversight.

Harmonization across Anglophone, Francophone, and Lusophone Africa is ongoing but remains incomplete while in South Africa, becoming a Medical Scientist follows a structured route:

1. Secondary School Completion

2. Bachelor of Science (3 years) with majors in biochemistry, molecular biology, genetics, microbiology, chemistry, physics, physiology, or anatomy

3. Honours Degree (1 year) in a relevant medical science discipline

4. Medical Scientist Internship (2 years) in specialized areas such as chemical pathology, hematology, immunology, genetics, virology, anatomical pathology, or medical physics

5. Submission of Portfolio of Evidence

6. Registration with the Health Professions Council of South Africa (HPCSA)

Impact of Medical Laboratory Scientists in Africa: Covid-19 Pandemic

At the beginning of the outbreak, laboratory experts identified the causative agent, a betacoronavirus later designated SARS-CoV-2 by the World Health Organization. Understanding viral phylogenetics and transmission patterns was essential to stopping its spread.

Across Africa, Medical Laboratory Scientists were responsible for:

  • Detecting and diagnosing COVID-19 cases
  • Validating testing protocols
  • Designing quality control programs
  • Monitoring hospitalized patients using biochemical and serological tests
  • Supporting vaccine development research
  • Advising on government containment strategies
  • Strengthening disease surveillance systems

They also supported sterilization and disinfection monitoring within healthcare facilities. Despite limited infrastructure, unstable power supply, shortages of reagents, and delayed testing facility setups in many regions, laboratory professionals worked under significant personal risk to maintain diagnostic services.

Global Health Security Index evaluations placed several African countries below global averages in laboratory system capacity, highlighting the urgent need for strengthening laboratory networks. The pandemic made one thing clear: laboratory systems are not optional, they are foundational to national health security. 

 

Challenges Facing Medical Laboratory Scientists in Africa

While Medical Laboratory Science in Africa has made strong progress in training, regulation, and regional collaboration, several challenges continue to slow its full development. These challenges are structural, economic, regulatory, and professional.

1. Infrastructure Limitations

Many laboratory training institutions and health facilities operate with frequent power outages affecting both teaching and service delivery [3, 4]. Without reliable utilities, sophisticated diagnostic machines cannot function optimally. In rural and underserved regions, transport systems also limit access to centralized laboratory services. The Global Health Security Index evaluation showed that laboratory system capacity in several African countries remains below global averages, reflecting infrastructure gaps that limit diagnostic readiness.

 2. Limited Access to Modern Equipment and Consumables

Outdated or non-functioning instrumentation reduces students’ exposure to modern techniques [3]. Even when equipment is donated, maintenance plans and spare parts are often lacking. This creates a gap between theoretical learning and real-world laboratory practice. Automation and molecular diagnostics are expanding globally, yet uneven technology integration across African institutions continues to widen training disparities.

3. Faculty and Workforce Shortages

A limited number of PhD-level faculty and subject experts affect curriculum delivery [3]. Heavy workloads and relatively low academic salaries discourage professionals from pursuing teaching careers.

4. Sustainability and Funding Gaps

Many laboratory training programs depend heavily on donor-funded projects. While external investments have improved HIV, TB, and malaria diagnostics, these disease-specific initiatives often create vertical systems rather than integrated laboratory networks [3]. When project funding ends, sustainability becomes uncertain. Government budget allocation to laboratory training institutions remains limited in several regions [3]. Without strong domestic funding, long-term stability is difficult to achieve.

5. Weak and Inconsistent Regulatory Frameworks

Although progress has been made, some African countries still lack strong regulatory bodies. Harmonization efforts through WHO-AFRO, the East African Community, and West African professional bodies are ongoing, but full standardization has not yet been achieved. Strong regulatory systems are essential to protect patient safety and professional identity.

6. Professional Interference and Role Encroachment

One growing concern across parts of Africa is the interference of other professionals, including microbiologists, biochemists, and science laboratory technicians, who attempt to assume core Medical Laboratory Scientist duties without completing accredited MLS training. While collaboration among health and science professionals is necessary, the independent performance, validation, and authorization of diagnostic laboratory results require structured professional training, internship, and licensure. Medical Laboratory Science is not simply a general science degree as we have explained thus far. Protecting the scope of practice of Medical Laboratory Scientists is critical to maintaining diagnostic accuracy and public trust.

Reviews highlight inadequate practical attachments and insufficient hands-on exposure in some programs [3]. When training duration is short relative to the content required, competency consolidation becomes difficult. Students need balanced exposure to both theory and supervised clinical practice to function confidently in modern laboratories.

 

Recommendations for Strengthening the Profession in Africa

To secure the future of Medical Laboratory Science in Africa, several steps are necessary. 

  • Infrastructure Development
  • Curriculum Harmonization
  • Strengthening Regulatory Bodies
  • Industry and Research Collaboration
  • Technology Integration
  • Workforce Motivation and Retention

 

Conclusion

Medical Laboratory Scientists are the silent backbone of healthcare systems in Africa. From detecting malaria under a microscope decades ago to running molecular tests for emerging viruses today, this profession has evolved remarkably. Yet, despite its impact, the profession faces serious challenges: infrastructure gaps, funding instability, workforce shortages, regulatory inconsistencies, migration pressures, and professional interference.

Strengthening Medical Laboratory Science in Africa requires holistic reform namely infrastructure investment, curriculum harmonization, strong regulatory systems, sustainable funding, and workforce retention strategies.

If Africa is to build resilient health systems capable of responding to future pandemics, non-communicable diseases, and emerging health threats, Medical Laboratory Scientists must be empowered, protected, and supported.
